Q. What are the procedures involved in filing a claim?
A. Life Claims:

A Claim for Life Benefits form must be fully completed by the Creditor and submitted to American National Insurance Company at 4500 Lockhill-Selma Rd, PO BOX 696785, San Antonio, TX 78269-6785. This documents the amount of payments made by the insured and the remaining balance required to pay off the loan. A copy of the policy, contract, and a certified copy of the death certificate must also be attached.

Accident & Health Claims:

Submit a fully completed application for disability benefits to the address or fax number mentioned above, along with the following statements:

  • Statement of Lending Institution
  • Statement of Insured
  • Statement of Attending Physician
  • Statement of Employer

    If disability results from either a motor vehicle or on the job injury, a copy of the accident report should also be attached.

    Q. How are policy/certificates and miscellaneous supplies ordered?
    A. Contact our Insurance Service Department at our toll free number, extension 7403 and have the form number(s) and quantity readily available for prompt service. You may also place orders directly from our "Supplies" link on our Home Page.

    Q. What is required to obtain a premium or cancellation quote?
    A. Contact our Premium Accounting Department at our toll free number, extension 7436 or directly at (281)535-7436 for premium quotes. You may also fax sample deals for verification to (409)621-7565.
    Cancellation quotes should be directed to our Customer Service Department. Please have the certificate/policy number(s) available, along with the cancellation dates.

    Q. Whom should you contact regarding remittance and billing issues?
    A. Contact the Premium Accounting Department at our toll free number, extension 7436 and have the account number and account name readily available.

    Q. How are rates and formulas obtained?
    A. Contact our Audit Department at our toll free number, extension 7414 or directly at (281)535-7414 for insurance calculation formulas. You may also utilize the Request Information link.
    Premium rates may be obtained online from the online procedure manuals or via the above mentioned methods.
